Restrictions: CAUTION: Federal law restricts these devices to sale by or on the order of qualified dentists, periodontists, or oral surgeons only.

These Terms constitute the entire agreement between the parties relating to the sale of products by the Company. No amendment or modification of these Terms shall be binding unless in writing and signed by both parties.
